Don't do it. If there is already health concerns running through your mind then dont do it. Problem solved. If he wants oral let the other girls lined up give it to him. Don't put yourself in a position to get sick/diseased because you think its fair that he get oral too. You dont have to do ANYTHING that you dont want to or are not comfortable with. You have no marriage license nor does he have any rights over your body and vice versa.

As to whether you should give your boyfriend oral, my answer would be, "if you feel like it, yes".
I think, however, that you should never give him oral just because he gave it to you. If he did that for you, that's his pleasure and yours; he should not "naturally" expect a retribution. The same for you: if you give him oral, do it because you want to, and don't expect him to give any "retribution".
As to the health risks, I understand you can catch AIDS, herpes, gonorrhea and bacterial diseases. If you want to give him oral, you should use a condom. You should not let his pre-ejaculation get into your mouth. And maybe he can bath before the occasion.

Get some strawberry condoms, there a bit of fun and you don't have to worry so much about infections. Don't do it until you are comfortable about it though, most men love oral sex but you shouldn't feel obligated to do it because he does it for you. In time you will want to do it to pleasure him. Have fun!
