 They were high class call girls or prostitutes. The term comes from the 16th century. Most were highly trained and educated to become kept women and playthings for all types of rich, important married men. They were only highly regarded by the men, however, and most would be socially snubbed and passed along from man to man, until they lost their charms and were married off to someone of minor wealth. Many didn't fare as well and wound up on the streets after their youth had flown away, along with their more wealthy customers.

A courtesan is basically a woman of low social rank that is kept around to have sex with the King when he feels like it.
Back in the day obviously all the high class women would not have sex before marriage. Plus you didn't marry for love as it was all arranged by someone else.
So you had sex with your wife purely to make babies to inherit the crown, but you had courtesans about so the King could have sex when ever he felt like it and it didn't really matter if a courtesan got preggers. She could chuck the baby in the river, or she could quit and go and live as a single mum. Nice kings would let them stay on as palace servants, nasty ones would chuck them on the street. Once they got old / ugly they'd be chucked out anyway and replaced with younger girls.
Nowadays there is no such thing, but if a guy is asking you to be one then it means he thinks an awful lot of himself and wants you as a plaything.
