I'm 19, but probably only look 16, and this knocks my confidence big time.Whenever i see a girl in a pub who looks nice, if i catch her eye, ill give her a quick smile, etc, but i never have the confidence to go over and say hello. I mean, what do i say? How do i know if shes even interested and im going to make a fool out of myself? Its something thats really bugging me, not having the confidence to go and speak to someone.

Hey sweetness,
DV1 gave you great advice, I just want to add in a little sumthinsumthin' about your young face. This, despite what you may think, is a blessing. Since you look young for your age, in theory you will ALWAYS look young for your age. So when you're thirty, you'll look like you're in your midtwenties.
Trust me, in a few more years, you'll be counting your lucky stars that you look young. And the ladies will be ALL over you. You'll have the maturity level of an oloder guy, but the face of a younger one. You're a double threat!!
As for talking to the ladies, as DV1 said, you have to grit your teeth and say hello. If you're feeling good about yourself, the girls will feel good about you. All it takes is a simple "hey" to get things rolling... if the girl isn't very responsive, it's alright. Just say "nice meeting you, have a nice evening" and move on. ALWAYS be a gentleman, even if you get tangled with a girl who's not very nice.
I think you'll be okay, sweetness.
Good luck!

First, looks are not so important for many people.
Second, you should understand that looks are made.
After reading your question several times, I got a feeling that you believe "a girl in a club" is superior to you.
You should think about her as equal to you, otherwise it will be difficult for you "to go over and say hello".
"what do i say? How do i know if shes even interested"
If you wanna meet her, believe that she is interested.

The worst they can do is say no. Sometimes, they'll get cocky, but just laugh it off, and realize that they're a girl from a bar, and no better than anyone else.
