But I really do not want him to just let himself go and me end up not attracted to him anymore. He already has bad self esteem i'm so afraid to tell him that I am not happy that he's gaining weight. He started working out a while ago and I always told him I thought it was so hot. I would even watch him workout his arm muscles and tell him how hot it was and he loved the attention and worked out in front of me all the time while I would compliment him. But then one day he was just like "I stopped working out I don't care anymore." How can I get him to start working out again ?

Well, a new year is about to begin and a typical resolution is to lose weight. Perhaps tell him something along the lines of that your new years resolution is to lose weight and get more in shape, that watching him when he was working out has given you motivation to start being healthier and that you'd like his help to achieve your goal.
After a while maybe ask him if he'd diet and excercise with you as some sort of support and do it together, that way he'd feel important as he would be helping you but also getting healthier without knowing your true motivations.

Maybe you two could start doing things together so that you can motivate him with compliments till he wants to do it on his own. You don't want to be blunt and just tell him he has gained weight, you want to make it fun for the two of you which should really help him along.

Tell him you are planning to start working out too(not like body building though) and you really want him to do it with you like a couple activity. That way you will both stay healthy and if you work out he might feel inclined to also.
