Would this be ok? He said he has never done it before. By the way that is all I recieved from this man on my birthday. A morning Happy Birthday wish,, hurt and confused

 Its perfectly appropriate. She's his extended family as they have children together.
What you need to think about is why you receive just a birthday wish from him on your birthday?? Is this indicative of the whole relationship and the way you are treated? Why are you not married after 7 years?
Just things to point out. It sounds like you are already having some doubts about what exactly is going on...think deep and hard and proceed with caution.
