 You can't go wrong with a bouquet of flowers. They make anyone feel like a million dollars on their birthday!
You could also get her tickets to the movies or to a play. Make her a mix CD (do people even do that anymore? It would be retro), or get her a fun bottle of wine. You could get her a cute accessory - can't go wrong with a cute pair of earrings, a necklace or a scarf.
You could give her the gift of adventure and buy tickets to a museum or a concert or amusement park a couple of towns away and then make a day of it going there. Take some pictures along the way... my Mom always says the best gift is an experience. I think she's right!

 Ay, wish I knew more about her likes and what-not but here goes nothing: concert tickets to see her favorite band (let her unleash some excitement :p ), avoid buying her make-up (not only a lame b-day gift but makes some girls think you think they need it-ouch!). If she luvs animals (and has space and time), a little kitten or puppy or a rabbit/guinea pig. Exotic perfume along with a scrapbook with pics of you two. Throw a surprise party. Get her a relaxing spa day and put the envelope in a box surrounded by her favorite candy (choco. truffles rock!). Get her the season dvds to her fav show. If you know her size, get her a stunning dress that will unleash her confidence. Something tasteful but made for the wearer to be noticed. Oh and if she luvs to read, a Kindle would be cool, put in her fav books before you give it to her :)
