I still love him though in spite of it all, and I haven't moved on. At first I thought time would heal it, but it hasn't. He hasn't moved on either...we talk every once in a while and he's made it obvious he wants to be back together, but I'm still scared of falling into the same old pattern. I guess my question is should I give him another shot again, do I really have anything to lose since trying to move on hasn't worked?

Well you still love him , and he still clearly loves you. According to your post, the relationship had ups and downs, so it wasn't like anyone cheated or was really nasty that it left the other hurt. Yes, give it another shot, and make it clear that things need to change from what they were. Make an effort to work through problems, and it should work out. If, however, it becomes like it was before, then end it and never look back. But since you both love each other, it might be worth another shot.
