I'm thinking more along the lines of if he finishes first and she's not ready to be done. Its easier then because she is already loose and wet. I have occasionally seen references to this happening. My wife and I tried it (admittedly, somewhat half-heartedly) a few times but it never came close to working.
Some women claim to have done it, and it was a unique and very intimate experience to feel the penis go through the whole cycle of erection, ejaculation, and back to flacid inside their vagina. In one or two places I have seen it suggested that this is the gentlest way to devirginize a girl. (I'm inclined to dismiss it as wishful thinking by somebody who has never ruptured a girl's hymen or stretched a virgin vagina open. I'd say it takes a very solid erection to do what must be done, not to mention that a young man who has a virgin partner will find it almost impossible to be flacid!)
It certainly can't hurt to try - at least a few times, building on what you learn from each attempt. Just guessing - it might be easiest to do in the "spoons" position, or possibly an "X" or "tee" position. Try it very early in foreplay, before you start to get erect, rather than after you go soft following an orgasm.

The problem you will have is avoiding getting erect whilst you are arousing her.
