If after such a long time of participating in oral sex and nothing has come up...then okay.
In principle though, it is the safest thing to have safe "any" sex. With oral sex....I would say genital warts would be a factor or any rashes or anything..herpes.

In principle, oral sex, as sex in general, does entail some risks. However, if you're both healthy and have been absolutely monogamous for four years now, I assume it would be close to impossible to get any sexually transmitted infection. I assume that both of you are healthy (except for the condition that forces your partner to take a medication) and you're like a closed loop.
Maybe there is a risk for transmission of other diseases. I assume that amoeba, for example, could be transmitted this way. But then, most likely they would be transmitted through your every-day life together anyways.

If you have been sharing oral sex for six months or more and no adverse symptoms have surfaced, then I would think that all is okay providing no additional sex partners are involved currently or in the future.
