I am so so sorry to hear what a tough time you're having. It's hard to answer this without more information about your circumstances.
My first instinct is to suggest you get some help. Visit your GP and explain and ask to see a councillor urgently. This will at least give you someone to talk to and should help with your depression.
On first reading it's tempting to say that your fiancee is obviously a very weak and immature person if he can't cope with you going through a bad time. However, there may be more to it than that.
It sounds as though you've been a very strong person in the past so I'm certain you'll find yourself again. Please write to me and tell me a bit more about yourself. Are the children his or from a previous relationship? Why did you have to give up so much to look after everone? What job did you used to do? I'll do my best to help you further.
