It could be possible but sounds unlikely but if you are really worried get a pregnancy test just to put your mind at rest.

Get an early pregnancy test and follow the directions. Speculation is a waste of time, but yes you could be pregnant. Depending on your morals you might want to see a doctor about an "after" pill.

It depends on when it broke. If it broke when he was inside of you, then some sperm could have slipped through from his pre-cum, but it is much less likely as I believe a lot of condoms have spermicide. And if you are sure all of his sperm came out after he pulled out, then you are more likely to be in the clear.
If that's the way it happened, I'd say the odds are unlikely that you'll be getting pregnant, but it isn't impossible as it only takes one sperm that can swim well to make you pregnant.
