 You have to suck so hard it will probably hurt a little. Couldn't hurt to bite the area a bit first too. Takes at least 30 seconds, depending on how easily the person bruises. You're bruising them, so you have to suck pretty hard.

 I sort of put my teeth lightly on the skin and suck strongly and then with my teeth still biting, lick the area and then suck again and then let go. I would say this takes me about 10 seconds :)

Once you have made the O shape place your lips on the skin and suck. Give it 20-30 seconds, and BAM you should have given him a hickey.
