I live in a somewhat different culture so I can't offer detailed information. The link posted by "CaringGuy" appears to be reasonable and complete.
The best advice I can give is to do one or two letters of application for practice. Send them to places where you might reasonably do your work experience, but not your top choices. Have a school counselor, teacher, or other adult who knows you (like an athletic coach, clergyman, or Scout leader) look them over and offer suggestions. After doing a practice letter or two you will have improved your form and style, and you will be more likely to make a good impression when you write the letter that really matters to you.
And if one of those other letters brings good results . . . you will be in a position to see if what you think you want to do, is what you REALLY want to do! And you'll get to practice writing an equally important letter, in which you sincerely thank somebody for his interest but graciously decline the offer. In the current economic situation, there may come a time when you need to re-open an offer you previously declined.

Go to this site http://library.bcu.ac.uk/learner/writingguides/1.06.htm
It offers examples of how to write a letter for this kind of thing. Hope it helps. And good luck.
