If they jump to conclusions then that says something about them, doesn't it? Don't let it worry you too much, let them think whatever they want. If they are decent people they'll not assume anything, but ask instead. And there's no shame in answering that you're nervous.

It's perfectly normal to be nervous on a date.
Why don't you be up front and just tell him you're a bit nervous and don't have much appetite? Good chance he might actually find it cute and it may break the ice a bit.
I would appreciate such honesty.
You surely can tell your date about your tooth but I hope that's a temporary situation anyway...
