first off, i wanna say i really really like u now cuz i know im not alone in the world for being shy :P
second, probably the best thing to do is to just talk to her, get to know her more, and if u guys happen to talk about the dance at all, ask her. If u guys rnt talkin about it n e time soon, u bring it up.
Just remember, be polite, deep breaths, and just relax. the worst thing that could happen is her saying no, but im sure she wouldnt think badly of u for it. usually its a compliment to girls when they get asked to the dance. But then again, im a guy, so im only basing this off of friends and my gf.

 Just go up to her, normally, and when in general conversation ask, soo you going to the dance with any1? if she says no, then say, well i was wondering, i havent either, would you wanna go with me?. Easiest way to do it, dont beat around the bush. Good Luck :)
