Me and my friends have the same sort of trouble, we go to an all girls school and so meeting boys is, as you can imagine, difficult.
Obviously, being stuck in the same position, I can't help you mate, sorry.

First, make sure you follow all hygene codes. Most girls at your age are more into looks than anything, so this is a big one.
Next, try to dress good. Look at magazine catalogs for references for buying clothes.
Next, don't try to be anyting you're not. A lot of people will notice when you're lying (unless you're a great liar) so if you start talking thing you're not/don't have, then when the truth comes up you will end up the bad person.
Next, always be yourself. I know, cliche'd, but true.
Next, never hesitate on doing something (except in a few cases). A girl likes someone who takes the bull by the horns. So, if you want to ask a girl out, don't hesitate, and if she says no, you won't blow up, so what's to lose?
When you aproach a girl, first look what she's doing/ looking at to start a conversation. For example, if you see a girl looking at a cookie, you could go up to her and ask her "So, I see you like sweets", or if a girl is playing with a soccer ball, you could aproach her and say "Can I play soccer with you?"
Good luck.
