After the snip they have to return for follow up tests to confirm that they no longer produce active sperm.
I persume you have heard of stories about women stating they are on the pill to their partner and really not been as they were trying to get pregnant, men can be known to do this too, I had a friend whos partner did just that told his girlfriend he had the snip and didn't really just wanted a way to not lose his girlfriend and wanted a family as soon as possible.

It is extremely rare. They ususally have to produce a specimen after their procedure to check that they are sterile.
