 are you role playing doctor and nurse or patient nurse? you could use toys or props to help but beyond that it really is up to you and what you're comforable with doing, if you're doing patient and nurse try something along the lines of "I need to take a sample of semen, would you like some help with retrieving it?.... proceed to give hand job/ blow job etc... just start simple and see what happens, he may like you to take control or may want to try and seduce his sexy nurse.
