 I'm going to sound really sad but what is second base these days? I'm going to take a stab and say you're girlfriend is rough when she gives you a hand job.
Talk to your girlfriend and ask her to maybe not be so rough. And I would go see your doctor if you are really worried about your foreskin being loose.
