The key difference is the wording...
If someone asks ARE you sexually active.
versus
If someone asks you HAVE you BEEN sexually active.
From a medical viewpoint the first statement is asking if you're having sex regularly or have had it recently.
The second is asking if you're no longer a virgin (essentially)- so the doc can rule out STD's, pregnancy and what-not that just isn't possible/normal if you've never had sex.

But in the pure medical sense - if a doctor asked you if you were sexually active - and you had been, but maybe a year ago (which you could still be suffering from diseases from that encounter) if you said you were not, that wouldnt really be correct?
Just my take on it anyway.

It means that you are no longer a virgin (you have had your first sexual experience).
It can also mean that you are having regular sex as well.
Even those who have not had sex in a while are called sexually active if they have had previous encounters.
