It basically means the person who holds the reigns and keeps everything together and going smoothly.
For example, my nan was the backbone of her marriage. Her husband worked and earned a crust but she organised everything, made sure bills were paid, made sure appointments were kept, cooked dinner and kept everything running like clockwork. She also kept down a job and just kept everything going. That isn't to say my grandad didn't, but my nan was definitely the backbone of the relationship, and its not a bad thing :)

To me that means the person is responsible for holding onto and maintaining the relationship. It doesn't always mean in a negative way, just a reference to someone being a strong character.

Well being spineless generally means that you can't stand up for yourself and that your a push over. Being the backbone of something means that you are the strength of whatever it is. So I guess whoever's the backbone of the relationship is the strength behind it, holds it up and is keeping it going.
