I asked a girlfriend this once and she laughed and then started to speak, but then declined to answer saying that it would ruin it for her if she told me. I am guessing my wife probably thinks about what china pattern would go best with the new curtains in the kitchen and how she could be painting her toenails. Am I far off with that guess, those who are married? As for me, I tend to focus on the act itself: what my partner looks like, what I am doing, how it feels, what it might look like if I could see the two of us from across the room. I also wonder if my partner is enjoying herself, if her eyes are open or closed, and if there is anything I can do to increase pleasure for us both or prolong the experience. Yes, sometimes I do try to distract myself with something else (like listing the capitals of states in my mind) in order to to last longer. I do not think about other people (past lovers or fantasy girls) or otherwise have fantasies like I might if I am by myself. What do you think about?

Ideally, nothing -- I just focus on us and magnifying love, passion & ecstacy in the moment. That only happen with girls that really turn me on and I connect with, though

There are many and varied things that I think about whilest having sex.... For example: I re-play a golf hole where I got a double-bogey..... or, a tennis point that lead to my opponent winning a set and match.... Sometimes, I even keep a 5-star Sudoku - which I haven't yet been able to complete - on the night stand, and review it to see if I can't get another number or two....
My G/F/sex-partner loves this because it focuses my attention elsewhere,.... and she's very bashful!!!!
There are LOTS of things you can think about besides the icky activity in which you are participating....
Good luck....

But you know yourself, sometimes we can be a bit distracted with other things in our lives and sometimes that kind of stuff can creep in. Especially if I wasn't in the mood for sex in the first place but giving her one because she was.
The vast majority of the time I'm completely focused on the act and my girlfriend.
I have to say though I would never ask a girlfriend what she's thinking about during sex. I'd rather she was lost in the moment and enjoying herself to actually be that distracted, if she was thinking about other things I'd rather not know because the ideal is she's feeling to much pleasure to think about the mundane.
