It depends on whee you're going. E.g if you're going bowling then jeans and nice top and jacket would be a good option. If it's to somewhere fancy e.g for a meal somewhere nice, a smart dress would be good. Also first dates are the time when you'll get to impress him. So you want to leave him with a good impression of you. Be yourself. Stick to your usual style.
I went on a date recently to a nice restaurant during the day. I wore a smart skirt and blouse (nothing too business like or granny like). This is my favourite work out which i've worn many times and know suits me. I feel good in it and people always compliment me.
You don't even have to buy something new. Just stick to what's in your closet and that you know you look good in and have worn before (you don't want anything new and uncomfortable either).

Wear something that is comfortable but makes you feel like you look nice. Don't go with new shoes or a new top. Ask your Mom for advice (she went on dates at your age once too) or just wear something similar to what you wore when he asked you out.

As for what to say, being nervous is normal. Remember to breathe (count to 5 once or twice to get your nerves back under control) and just be yourself. The best friends and boyfriends I ever made were when I was just being myself, not trying to show off or impress them. Relax. You'll be fine. It's just two people meeting up to get to know each other a bit better.
