Ultimatum: Answer tonight or done forever

My boyfriend and I have been together for over two years now. For the past year it has all been long distance and it has definitely been trying. We have visited each other a few times during the past year, though. Anyway... We keep going through periods of breaking up and getting back together and then arguing again and starting the whole process over. Whenever we are physically together things are wonderful and I love him more than I ever have imagined I could ever love anyone. The distance is really playing its part to spoil things for us though. We hardly get to talk and the lonliness is difficult to bear. Not to mention my bf's insecurities and jealousies towards me and my nagging doubt that maybe he is just not the one. If we can just stick it out for a few more months, he'll be able to relocate to my area and we can actually be physically together again. But if I am not sure of us, I can't ask him to move halfway across the country just to try and see how things go. He needs a little more commitment than that. So, this morning I told him I wasn't happy with our relationship right now. He said he thought we should break up. I went on about my day for a few hours and was very upset and then sent him an email saying I don't know if I can do this and it hurts, etc. He emailed me back saying he wants to know by the end of the day if I want to be with him, or he is done forever. I honestly don't know if I should get back with him. Then again, I am very scared to lose him. I really can't picture a future without him. But we can't kid ourselves into thinking that all of our problems will be solved if he moves over here. What should I do? I'm so confused! It's giving me a massive headache!
