This is one of those things that it is hard to answer in general because each person would have a slightly individual answer so prehaps you should bear that in mind.
1)It is mostly people having sex though there is often some, usually lame, storyline contextualising it if it is a film. Generally speaking 'soft-core' tends to have more 'story' content than 'hard-core'. Erotic writing often has a plot too.
2)I guess the point is it's fantasy and fantasy does play a role in our sexual desires and our existence as sexual beings. Of course, if you are horny and single then the 'point' can just for it to be a stimulas. One of those questions that would have a slightly different answer from person to person.
3) For a variety of reasons; voyeurism is something alot of people find erotic. For example, husbands (and wifes to be fair) sometimes fantisise about watching there partner being done. Some men fantasise about watching two women. I guess the point is as above; its fantasy and fantasy's can be powerful and stimulating.
4) Spekaing personally then the answer is oh yes! But seriously doing it yourself isnt quite the same experience so some people would answer no to that. Its not a question of preference; its a question of different experiences.
