Ouch that is some serious PMS! I know you're upset and probably fed up too. Have a long chat with her and try to sort it out. Talk to her and explain how you feel, how you want to help her. And if it just gets too much, try not to spend so much time together during her period. Or get her to see a gynaecologist and see if they can treat her with something to lessen her mood turns and swings.

If you know it's PMT, and that she doesn't really mean it, why are you
a) getting so upset about it all
b) being around to annoy her enough to make her dump you.
If the way you breathe is enough to make her want to end it then don't breathe around her.
When it gets to that time of the month then go away, leave the house and go out with your friends, leave a supply of chocolate and flowers behind and run like the wind.
Good Luck!! xx

You should sit her down and talk to her about it but I would give it one more chance but if she ends it again you should leave it at that, you can't keep having a relationship with someone if it's just going to keep ending over and over again. She might not really care about your feelings if she keeps doing that so just talk to her and let her know how you feel and if she feels the same, let her know that you want to try and make it work well good luck.
