It can sometimes happen and it is due to human error. The human mind is sometimes too busy and his mind may not recognize your voice at an instant. This is quite normal .

Different phone lines, (landlines , voip, mobile) can give slight and subtle differences to the voices carried over them. You may just not sound quite like you.
I cant count the amount of times in phone calls I have thought a friends son was the friends husband, yet, I would never mistake their voices in person - they don't even sound alike imo, but on the phone - clueless.
Not recognizing a voice tho is hardly grounds to get suspicious and decide it is due to other women calling him.

He may have insensitive ears to the different sounds of peoples voices. I wouldn`t jump to conclusions. It took me months to be able to tell my gf and her sister apart on the phone, they would play tricks on me because of it. Also different lines produce different background noise and he may also be experiencing some hearling loss where the speach frequency is located due to loud music. Its a common problem with alot of people.
I wouldn`t worry unless he is totally not into the conversation, but voice recognition is nothing to fight about. He could do that to his own family all the time too.

Usually it doesn't matter how many dozens of people call you you are able to distinguish each and every voice. Perhaps he has some kind of ear problem that prevents him from doing this. I'm a bit confused about the different line bit. Is he able to tell who you are on certain lines but not others?? If that is the case maybe there is a problem with line quality on those particular lines.
