This is why you hold the base of the condom ON while he withdraws. Otherwise, you are going to risk spillage!
Then what would be the point of wearing the condom?

 i know someone this happened to and it was coz they didn't know it had fell off! afterwards they were like 'WHERE'S the condom?!' and then they realised. if this is about yourself you will need to get a pregnancy test and if a guy has done this to you it shows he is disrespectful to just leave you with gross article to get rid of (without even saying anything?)

For what its worth, my guess is that after ejaculation, he went limp (naturally) and when withdrawing, the condom slipped off and stayed put.
Possibly a situation where a guy would feel too embarrassed to discuss it.

Why would he leave it? Probably partially for the same reason some men leave socks on the floor or wet towels in the bathtub. Because they don't clean up their messes.
Another reason is because the girl is freaked out and livid the condom fell off, which makes it difficult for them to relax and let the guy "dig" it out of them.
Another reason is because they're embarrassed it slipped off, and they might have gone ahead and finished inside her, in which case, they bolt as fast as they can.
That's a tough question.
