He always says "i love you" to her and tells her she is beautiful, sexy, hot etc etc. Also he says that he enjoys spending time with her not just a sexual relationship. What is the actual deal here? Is he just saying all this stuff to please her or does he actually love her?

Of course he's saying it for sex. He couldn't care a less about her at all. Married men rarely leave their wives. He's just using her. If he loved her, he would be with her. Your friend is really wasting her life.
