I would steer away from poems: unless you're a pro at writing them, they can come across corny. Stick to a really meaningful and heartfelt letter. Say everything you really want to say while you have the chance.

Well I can try. The first important thing is to remember you are crafting an expression of how you feel; so prim and proper grammer and rhyme is not essntial. You need to think about what is special to him, what is special about you both as a partnership, how he makes you feel, special times you have shared.
Pick up on little things he does and says, things that are unique to him, mannerisms maybe. Anything that is distinctive....unique and special about the love you share...
