I once again have to agree with Ginalolabridga, I think any female would feel extremely touched by this.
What have you really got to lose by sending it, you have said that once things seemed to be on between the two of you she backed off. Was it guilt that maybe she feels something for anyone else or the fact that she can't believe it is happening or that it is true.
By sending your poem to her on her special day she will know your true feelings and can make a decision either way. At least you may get a reaction either way and I don't think anyone could be annoyed at receiving such lovely words. They are truly from the heart and I just hope that things work out for you.
