Though it has been many years now and we are hardly in any contact now, live in different cities also, I still can't think beyond him and feel that I should give it a chance at least. I know he is single. But since I am so very much interested in him, I am scared at the very thought of... what if he refuses me? I can't rather should not go on like this with my life. I know, but still can't gather enough courage to say it to him... what should I do? How should I approach him on phone?

My observation is that men are much smarter to pick up "Something more than friends" vibes than women. But I would request you to just ask him out for a coffee casually where you both are alone. Talk to him that you think a lot about him and do like him. Try to see if he has similar feelings for you.
If he says, I haven't thought about it or as such, just move on. No point in having a one-sided feeling.
If yes, make merry !
