Obviously its better if you can get it out because there is more freedom for movement but sometimes this isn't possible for whatever reason. I don't think the guy would have been disappointed because after all, he had a girl touching his penis and he would have understood about the trousers-on situation. A method you could try if you can't 'get it out' and have difficulty giving a hand job with the trousers on, is (after making out for a while- getting heated etc) sit him on the edge of the bed or wherever, straddle him (legs either side of his hips) and grind yourself against him up and down. The rubbing alone and the mimicking of sex should be enough to get him off and it can also feel good for you if you are wearing loose trousers. And the bonus of this? My advice would be to go somewhere private, get it out and do it there. It's really not possible to give a good handjob whilst the guy has his trousers on and done up. Also, it gets the insede of his trousers really messy, if you know what I mean. Not good at all.
