 is she on facebook? are you? can you contact her that way?
does her family have a landline you can look the number up?
and I agree having a backup of your phone is a good idea.
if you have no other way to contact her go see her.
BTW, do you always do all the work in the relationship? that's not a good sign.

 If you can't go over to her house, email, or message her via fb, even consider posting a snail mail card. She may be waiting for you to contact her, she might also be stressing over not hearing from you, she may be tied up with family stuff .... and she might have broken up with you, but you wont know what the story is unless you contact her.
And next time back up your info, keep a list somewhere of all the important contacts to save you this sort of stress happening again in the future.
