You're in Canada, right? http://www.cfsh.ca/Members/Find_a_member/default.aspx
You have to be your own health care advocate. No one else is going to be as diligent in finding health care for yourself.
You did NOT see a "lady doctor" in Canada, not a properly qualified one, anyway, if she didn't examine you.
If you are Canadian in Canada, I believe that you have the right to medical care without paying cash. So get yourself to the nearest clinic (from the center finder tool I provided above) and GET YOURSELF SEEN BY A PROPERLY QUALIFIED DOCTOR NOW. Having a bunch of amateur strangers on the internet give you folk remedies is a recipe for disaster. Stop taking silly chances with your health and get going.

The best thing I could tell you is get a second or third opinion and never return to the first doctor again. How can she not examine you? Wow, that is just so messed up. Oh, please please seek professional help because what is happening to you sounds so so bad! And if you believe in Our Father, pray to Him as well!

If it's a pain in your cervical area then the Gynae is the best person to give you advice. Paracetamol is always a good place to start for pain relief, but you can usually ask your pharmacist for some free advice. If your urine is dark then you might want to consider drinking more- you should be drinking about two litres a day. Hopefully the Gynae doc will be able to enlighten you a bit more, even if it's just to reassure you that there's nothing wrong.

Did you read about TSS? Read it again. And get yourself to an emergency appointment ASAP. Don't wait for a doctors appointment. Just go!
"Most associate TSS with the improper usage of tampons, which can cause immediate and life threatening illness when tampons are left in for too long."
"The symptoms of TSS develop rapidly. They include a FEVER usually higher than 102 degrees F. (38.88 degrees C.), a RASH covering the body, and severe VOMITING or DIARRHEA. One may also be very LIGHT-HEADED or feel FAINT, caused by a significant drop in blood pressure. The eyes, or the vagina may appear brightly red, and the person affected may show confusion, or have a severe headache.
The presence of several of these symptoms constitutes an emergency, particularly if one uses tampons"
EMERGENCY! GO RIGHT NOW TO AN EMERGENCY ROOM!
"Treatment required is immediate, usually intravenous (IV) antibiotics to stop the illness from spreading.
Failing to treat TSS can result in severe kidney infection and ultimately death; thus, signs of this illness should never be ignored"

This is not me joking around with you. I am quite serious. By the sounds of it you could have TSS, and you NEED TO GO TO A DOCTOR! Explain to them your symptoms, and about the tampon, and they will know what to do.

Quit being ashamed and get to a doctor. I know I'm a man so know nothing about most of all this. But you might have an infection or something.

You have to see a doctor. The smell is a good indication that you didn't get all of the tampon out. The smell probably comes from discard that has been sucked up in the tampon and does not get out. Leaving up inside of you can be lethal. You could die. Read up on Toxic Shock Syndrome: "Men, women and children can get toxic shock syndrome, for example following burns, boils, insect bites or infections after surgery. About half of the reported cases are linked to women who use tampons; the other half are not."
"TSS resulting from infection with the bacteria Staphylococcus aureus typically manifests in otherwise healthy individuals with high fever, accompanied by low blood pressure, malaise and confusion, which can rapidly progress to stupor, coma, and multi-organ failure."
You need to see your doctor. The information is described in the leaflet that comes with your tampons as well. Read it!
Read this if you can not find the leaflet: http://www.toxicshock.com/tssfacts/index.cfm
You need to take this seriously and get yourself to a doctor. You are scared, alright, but this is something you have to suck up and do even though.
Of course, you could also be fine once you get the leftover tampon out, but the longer you leave it there the more you are pushing your luck. Don't sit back and take the risk. Tell your husband what is going on and cut back on other expenses if going to a doctor is too expensive. Your health is more important than anything else you own. Sell things if you need to. TSS is lethal, and you are running a higher risk of getting it when you leave a tampon/parts of a tampon inside of yourself for this long. I'd rather you went to a doctor and found out you are fine, than to not go and have it reveal things were not fine.
