(Unless someone else has access to his phone and is trying to cause trouble) I would guess, not knowing anything about either of you, that A - He is not very intelligent and thought you wouldn't see it or figure out how it got there or B - There is a third party who would like to see some trouble in your relationship for whatever reason.

I think you should trust him on this too. Someone pulled a prank on him. You know your man, so is he stupid? You wouldn't marry a stupid man right? If he is not stupid he didn't do it. It is facebook... why on earth would anyone with brains do that. And even if he did while drunk or whatnot, it must have been a joke.
I vote for letting it slide. Unless this happens 2 more times. The 3rd time something happens is when you are allowed to bring it up.

I've done it to numerous people it's great fun. I once stayed in a friends place and his mom had her facebook password written on a notepad beside the computer and he had his account logged in. I couldn't believe my luck.
I changed both their relationship statuses to 'it's complicated' to each other (a mom and her son haha), I then changed both their profile pics to the same pic of them two together with his father but I cropped out the father and then posted statuses on their profiles to the effect of 'it's about time people knew how we felt'
They stayed like that for about an hour before he found out, the amount of messages and things they got was hilarious. They're a family of practical jokers, his mom even left hers like that while she phoned people to look at it and have a laugh.

hmmmm, it dont sound like something he would do, knowing you are able to get onto his facebook photos, I reckon he has been hacked, or someone has got his password and is winding up, unaware of the consequences, or not caring.
Has he used someone elses phone to look at his facebook account, has he sold a phone that may have stored his facebook details, I've signed into facebook on my phone once, 8 months ago, and daily i go on, and never had to sign in again, its stored/
You have got to ask yourself, if he done this, knowing full well you'd see it, why would he be defending his innocence, it dont make sense.

Well would he really put it on facebook if he knew you would see it ??? He must have known you would see it ??? The question is what is it doing on his phone in the first place ?? Did someone have his phone and loaded it on for a reason ????

His account could have been hacked, or he could be lying. Has he tried to log into his account? I have had a couple of friends who's accounts have been hacked. It's pretty common. He'll know for sure if he can't log in anymore.
