I once worked for this company, I asked after 90 days,for a raise and was denied, I didn't ask again for 2 years. When I finally asked after 2 years they said of course and gave me the maximum+. When I asked them why I got so much they said you have seniority. I said then why didn't I get it earlier; they said "you didn't ask". Point being "nothing ventured; nothing gained". Get off your horse and ask her politely. You could even joke around about it and say something like "You remember that time I asked you out"? he he "Have you changed your mind yet"? "What are you waiting on Christmas"! Or there's the subtle approach. I'm going friday to this movie I heard is cool, wanna hang out with me then? However you do it: JUST DO IT. One day if you don't, she'll be gone, you'll be gone; whatever the case might be and you'll never know, what might have been.

Try being her friend first. She already turned you down once. You don't want to come across as being desperate or pushy.
