Following up what Person12345 is saying, when I'm getting a pelvic exam, I'm always a bit tense, but my doctor always tells me to place my hand over my stomach and take deep breaths, then let it all out, take a deep breath, let it all out. Still feel a little discomfort, but it really does relax me. And my doctor is super-patient and cool with it all, as she tells me what I'll be feeling ("you'll feel a little discomfort here") and makes my comfortableness paramount.

If he/she didn't ask that was very unprofessional and you should have brought it up yourself. In the country where I am from, virgins are not examined. If you have some problem they check you with an ultrasound.

Yes, it's possible, though it's more likely that you were just very very tense when she inserted the speculum and that's what caused the intense pain. Generally the hymen actually has very few nerve endings. The pain many people feel from a speculum or first time sex is not the hymen breaking but stretching an unprepared vagina, which hurts WAY WAY more than a hymen breaking. Bleeding after a pap smear is pretty normal, I always bleed/spot a bit after I have them.
Next time try to relax first. If you're clenched and tense, it's basically like, if you've ever had a foot cramp, trying to pry that out of cramped position, which hurts immensely. So try to take some deep breaths, relax, breathe. If you are finding it impossible to relax, a doctor can give you something to take a few hours ahead of time to ease your anxiety about it.

The hymen can be broken during lots of things, not only for sex: playing sports, riding bikes, even riding a horse can break the hymen. And yes, during pelvic exams.

Yes it is very much possible and am pretty sure this is probably what has happened. It might be a bit uncomfortable but it will heal in no time, really you should have told them that you where a virgin so they could have warned you about the pain and bleeding but no harm done. Just because your hymen is broken does not mean that your virginity is gone, you are still a virgin and you always will be until you have intercourse so do not worry about it.
