ha-ha, I just replied on the role-playing suggestions question with this very fantasy! It's not that she wants to be raped in real life by a stranger by any means. She wants her boyfriend to do it - obviously she knows him and feels safe with him.
When I was married over 20 years ago, my husband and I had great fun playing out this fantasy. It was extremely erotic.
It's a gentle form of dominance without getting into hardcore S&M, if you're not into that sort of thing (and I'm not!). I think I can understand your situation but to quote Sigmund Freud, "The only unnatural sexual behavior is none at all." So as long as people are fulfilling their fetishes in a safe way, then I won't pry into it too much. Would you believe there are people who get turned on by playing with/eating feces? Fetishes can be almost anything and is different for everybody. We all have our own fetishes, kept secret so people wouldn't judge us. Your sister told you because she trusts you (very much so), most people would not even think of telling family members what turns them on! If you tell her she's mental then it may break her trust in you and she may never tell you things again, even things that may be harmful to her (abusive husband for example).
