They can be transmitted just by mouth to penis contact. Even if he does not cum, the penis still releases fluid, in the form of pre-cum and STD's can lurk on the skin (think genital herpes etc). If you had a cold sore, you would transfer this to his penis, and vice versa.
The best way to avoid this unless you are 100% sure he is std free is to wrap it up.
