"Blue balls" or "stone aches" really only happened to me once, long ago, and that was enough. I make certain to never let it occur again. The ladies basically have it right - if a guy is aroused almost to the point of ejaculation, but for some reason does not finish, it can be almost as painful as being kicked in the scrotum. I don't know if it happens to all guys, or if as painful, but as I said, once was enough for me.

Yeah, qcumbr is right - it basically happens because his balls swell up with sperm ready for it to be shooting out and then when it doesn't go anywhere it makes them ache.
You might want to introduce hand jobs to your boyfriend... or just give him a little bit of time to himself in the shower to give himself a bit of relief.
It won't do him any harm but it's not nice.
It's exactly the same as when women give up breast feeding and their breasts get all swollen with milk.
