A transexual can be a woman who dresses as a man but it is more often used to describe a man who dresses as a woman. It is not a birth defect its a choice in life. So to answer your question if they have the penis they have the balls and can cum. However some transexuals have sex change opperations so the have what looks like a vagina and a girl can have a sex change opperation that enlarges her clit and makes it look like a small penis and then the sew up the vaginal lips and insert the same stuff that they use for a boob job only in small bags to make balls and then they look more like the opposite gender.
