Why not smile more? Smiling actually tenses all your face muscles which shouldn't make you feel like you're putting on an act, while it'll also make you look relaxed.

A face transplant is a bit extreme!
The best way I can think to change this is to stare at yourself in the mirror. See what your face looks like when it's relaxed or when you pull an assortment of facial expressions. See what it is that makes your friend think you look nervous all the time, and figure out whether there's anything you can do to minimise this. Work on this, it might just be training yourself not to furrow your brows, or etc.
Of course, it's all just in the eye of the beholder, and if your friend is the first person to have ever mentioned this to you, then it's possible that no one else has ever even noticed... I mean, you obviously didn't notice until they told you, and you look at yourself everyday in the mirror!
Just try to relax. I'm pretty certain that you're not putting guys off.
