There is no difference between orgasming and coming (or cumming) for women. For men, it is possible to have an orgasm without ejaculating--usually through prostate stimulation--which could technically be called orgasming without coming, I suppose.
But, back to your question. I think it is more likely that rather than having orgasmed, your girlfriend is becoming worn out or sensitive without having an orgasm. It is a delicate balance sometimes, between having enough stimulation to come and having so much stimulation that you become sore or numb. If she isn't making any sounds or signs that she has had an orgasm, I would assume that she is experiencing the latter.
Best solution is to ask her. Even better solution is to get her to tell you exactly what she needs and wants to orgasm. Even better than that is to ask her to show you what she likes. Then follow her lead.
Real orgasms are pretty easy to spot. If you're not sure--ask. Far better to talk too much in bed, than not enough.
