 I would ask her. I don't know much about open relationships, but if it were me, I know I'd be shy about asking to be with another guy again. Maybe she thinks one time meant one time only, and she doesn't want to hurt you by asking?

Personally i would just say something like "would you like to go away for your birthday? or maybe a present like last year? ... i want it to be something special darling".
Whatever her answer (although you would like her too) smile and accept it.
