It did not last longer though and she is now single again. I was at her birthday party where I found this out but I was hesitant to make a move. I am not sure now how to approach her and maybe reignite the chemistry we shared. How do I find out if she would like to pick up where we left it.Thanks

Plain and simple...talk to her and see where she is in life, and what she actually wants. For all you know, she might not want a relationship right now (in any case you dont want to be the rebound guy), just take her out for a cup of coffee, have a chat, and take it from there.
Wish the best for you! Hope it works out!!
