Old feelings die hard, should I bury them or take a chance?

I have been in a relationship with my boyrfriend for almost a year now, and he has done so much for me. My issue is that I cannot get over this guy who I have had a GIANT crush on my whole life, or since 5th grade at least. Literally, 5th grade when he walked in to class on halloween with a skull mask that dripped blood. I have never told him how I feel. He is talented, and smart, and funny, and so fine. I am fine as long as I don't think about him or talk to him but I still do. I know we'd be good together we click when we talk and hang out. my boyfriend is almost ten years older than me and he smokes and though I love him I'm not so sure anymore that he's a good mate.

The other issue with this unbelievably wonderful guy is that he lives a few states over and he's going to live in England for all of next school year, what should I do?
