I highly doubt you saying "you have terrible teeth and your breath smells" is going to go over well.

Grown-ups are totally aware of bad-breath and body-odor, and why it happens. If you don't wash or brush; it only goes you will smell! You shouldn't be shocked; if you are offered a mint, or someone gets up and takes another seat on the train. Or, if your co-workers scatter when you sit-down!
You are intimate with this woman! Offer her the name of your dentist for a cleaning; and suggest to her that good oral hygiene is important for good health. Bad teeth can lead to gum-disease, and even heart-disease! I'm not French-kissing with a nasty mouth! Never!!!
Go out and by her a new power tooth-brush, a good tooth-paste, and mouthwash. If she is insulted by that, she should be even more ashamed that she doesn't take care of her oral hygiene and flaunts her bad-breath and yellow teeth all over the place.
If you love people, you have to be honest with them. Is it better to avoid kissing her? How do you think that makes her feel?
The general consensus is to be honest. Now go tell her! She is embarrassing herself!

The only real answer is a truthful one.
Tell her that she needs to see a dentist, as there might be an infection that's causing some halitosis (bad breath), and that you want to be able to kiss her. If she gets defensive, which might or might not happen, stand your ground, and tell her you love her enough to be honest, that you want to kiss her, but her breath is killing you, and it's very fixable!
