Well, to simply answer your question - she needs to be versed on the signs of a man cumming in order for her to be able to know what's happening.
The man, usually will have some sign of cumming. Increased breathing, tensing up or grabbing something nearby like the side of the bed, clenched fists, curled toes, the balls might even start to draw up near the body (like when they get cold). If you're a verbal person during sexual activity, meaning you do something like moaning - then your moaning gets louder and more frequent. And for some guys, it's even a reflex for him to actually say "I'm cumming!" - he's not meaning to say it, it just comes out.

Yes there is, if a guy is coming inside a girls mouth, you may not realise but the man always starts to move more, his hips, legs, feet, his heavy breathing will stop, and there will be silence, before a orgasmic groan as he comes (usually i find, from experience) as a man is about to come the girl can always feel throbbing from his penis and the semen coming along the tube creates a pulsating that the woman can feel.
