Are you worried about a disease or being found out that you are sexually active? A principled doctor will not reveal anything about your medical situation to anyone, unless you are underage? Or have some sort of health issue that would require reporting, such as a reportable STD?
What's happening that has you so worried?

Is there a reason you don't want your doctor to know you are sexually active? It's important information for them to know for treating you and what sorts of tests and exams they order. A doctor can usually tell if you've had sex from a pelvic exam, whether you have a disease or not.

Nope.
I doubt there's such thing...but there's such a thing called shame that gets in the way of you talking about a sexual disease or feeling free to be honest with a doctor.
If you think you have a remote chance of having a sexually transmitted disease, please see a doctor and please be completely honest with him. The health of your partner and you depend on your honesty.
The only outlying think I might think of that could get worse with sex is a yeast infection, but it isn't necessarily caused by sex.
